This dental examination must be completed by a licensed dentist on or after 11/15/21 and be documented on the IDPH Dental Examination Form. Class of 2026 Families : All incoming freshmen are required to provide:Ģ) Dental examination by no later than May 15, 2023.Targeted communications are regularly being sent to families that still need to submit their child’s birth certificate. A copy will be made and the original will be returned to the family. Those families that have not yet submitted a birth certificate need to do so immediately. The birth certificate must include the name of at least one parent. Submit Birth Certificate (Incoming Class of 2026 students and new students) : Birth certificates are required for all students. The deadline for families of incoming freshman students to submit an original birth certificate to the South Campus Main Office was July 18.Complete Online Registration : The deadline for completing OLR for the 2022-23 school year was May 27. Those families that have not yet completed OLR need to do so immediately. Details are outlined within the Enrollment Services tab on the LTHS website.
